Discharge Medicines Service

When you leave hospital, your medicines may have been started, stopped or changed. With the NHS Discharge Medicines Service, the hospital can send details of these changes to us, so we can check your future prescriptions match what was agreed in hospital.

How we help

• We review the information sent by the hospital
• We check your next prescriptions reflect any changes
• We talk to you about your medicines and answer your questions
• We contact your GP practice if anything needs clarifying

Getting referred

Ask the hospital team to refer you to your usual Smart Pharm pharmacy when you are discharged. Make sure we are your nominated pharmacy so your prescriptions come straight to us.
